Our dedicated Board of Directors and our team of volunteers make it possible for AVHN to continue its mission of keeping Worcester-area individuals and their families safe, healthy, and independent at home. Our team of dedicated professionals is committed to meeting the unique needs of each patient and their family with an individualized care plan. Under the Auburn VNA Health Network umbrella, an individual can benefit from one of the Agency's divisions or all three. Through a continuum of care, we work with you as a partner in your loved one's ongoing and ever-changing care needs.
